Band Boosters Membership Form	Join the Band Boosters!
"Pick your Pleasure" Volunteer Form	Form listing a number of specific ways you can volunteer to help the band.
Tax Credit Deposit Form	Form to submit in connection with your tax credit donation.
Liability Release Form	Form to be filled out and presented to band staff 24 hours ahead of time if you plan to drive your student home from a band event.

Band Physical Forms	A sports physical is required for all Marching Band members. One way to get a physical is through Physico. This link takes you to the Parental Consent form and an AIA Sports Physical Form.
CdS Discipline Agreement	Discipline Agreement to be signed by student and parent/guardian.
Marching Band Glove and Shoe Order Form	Form for ordering gloves, shoes, and other marching band items.
